(P208) Monkeypox among People Living with HIV, Chicago, IL June 1-November 30, 2022.

Background: A global outbreak of Monkeypox (mpox) was first recognized in May 2022. The percentage of people living with HIV (PLWH) & co-infected with mpox ranges between 36 and 42%. The objective of this study was to conduct descriptive analysis for mpox/HIV co-infected individuals living in Chicago.

Methods: Surveillance data for mpox cases reported to the Chicago Department of Public Health (CDPH) via Illinois National Electronic Disease Surveillance System (INEDSS) and case investigation data reported in REDCap were matched against enhanced HIV/AIDS reporting system (eHARS) and analyzed using SAS 9.4 version.

Results: In total, 1109 cases of mpox were reported between June 1 and November 30, 2022. Overall, 43% (N=476) were coinfected with HIV. The median time since HIV diagnosis was 9 years (IQR: 5-15 years). The median age of mpox/HIV+ individuals was 36 years (range 18-69, IQR=13.5) in comparison median age for mpox/HIV- individuals was 33 years (range 18-66, IQR=12). Approximately, 44% of mpox/HIV co-infected cases were among non-Hispanic Blacks, 23% among non-Hispanic Whites and 29% were among Hispanics. While 17% of mpox/HIV- cases were among non-Hispanic Blacks, 47% among non-Hispanic Whites, and 31% among Hispanics. Among 476 mpox/HIV + persons, 88% (419) received HIV care in the last 12 months, 65% (309) were virally suppressed, and 16% (75) had a CD4 count < 200 cells/ul. Hospitalization status was known for 918 individuals. Among 74 hospitalized individuals with mpox diagnosis, 66% (49) were HIV coinfected. Among the 49 mpox/HIV coinfected 47% (23) were not virally suppressed and 25% (12) had a CD4 count < 200 cells/ul.

Conclusion: The risk of progressing to severe disease is higher among mpox/HIV co-infected individuals with low CD4 counts. It’s critical for PLWH to remain in care and ensure adherence to antiretroviral therapy (ART) and viral suppression to reduce transmission of HIV infection and any adverse effects of mpox infection.
